Who can receive health information? Anyone with proper authorization from the patient, parent or legal guardian. -
How can someone obtain a copy of his/her health information? The patient, parent or legal guardian must sign a Release of Information form that authorizes Doctors Hospital to release copies of his/her records to whom he /she has authorized. -
What is the turn around time for requested information. Information will be mailed out within 4 working days. -
How would a Primary Care Physician obtain a copy of a patient 's record? Contact the HIM department to have the record mailed to them at no charge. -
Is there a charge for copies of the record? For patients, attorneys and insurance companies: a.-$35.30 pages 1-10 b.-$ 1.18 each page 11-60 c.-$ .59 each page 61-400 d.-$ .30 each page thereafter. Additional charges may apply for the cost of mailing, shipping or delivery. -
If I receive an authorization, should I release information to the requestor? No, forward the authorization to the HIM department for processing. -
