Inpatient Insurance Verification – 1^st floor, room 162 

Newborn Registrations - Emergency Dept  (located in Basement)

OB Admits/Observations - Emergency Dept  (located in Basement)

OB Pre Registration - Discharge Office, Room 164  (1^st  floor)

Outpatient Registration -  (located in Basement) for OP tests including lab, radiology, day surgery/endoscopic/EKG, stress testing, same day scheduled admits, pre admit testing

Pre Registration – 1^st Floor, room 162

Sleep Studies - Emergency Dept  (located in Basement)   

Quest Lab and Lab Corp patients do not need to be registered through our facility and may proceed directly to:

        Quest Lab located PBI

        Lab Corp located PBII

Financial Counselors – 1^st Floor, Room 164:  Payment arrangements, payments for copays, deductibles, deposits prior to services and current bills, assistance with billing questions, turn in insurance information not available at time of service

Patient's' social security number

Date of birth

Mailing address

Phone number

Name, address and phone number of nearest relative

If covered under spouse's insurance, insurance card for spouse, spouse's date of birth, employer's name
